What is a PCMH?

The National Committee for Quality Assurance (NCQA)  Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients' experience of care and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits. Each patient's care is overseen by clinician-led care teams that coordinate treatment across the health care system. Research shows that medical homes can lead to higher quality and lower costs, and can improve patient and provider reported experiences of care.

Is Physicians at Sugar Creek recognized as a PCMH?

Yes.  We have been recognized as a Patient Centered Medical Home since 2011.  We are currently a Level 3 PCMH, the highest level of PCMH achievement possible.  The recognition lasts 3 years, hence this year we have been working diligently to gain approval as a Level 3 again.   If approved, we will not only continue our Level 3 status for the next 3 years but do under the stricter 2014 guidelines.
