How do I make appointments?o

We offer you the ability to make, reschedule and cancel your appointment at any time using our online website services. You must be a member of the website to use this service. If you prefer calling, please contact our office at (281) 325-4100.

What is a Wellness Visit?o

A wellness visit is a once-a-year visit to our office for a general health check. This is often referred to as a “physical” or “checkup”. The goal is to help lower your risk of illness or injury. During this appointment you can expect your physician to do the following:

  • Review your health history
  • Review your list of current medications and supplements
  • Update your family health history for any new serious illnesses
  • Measure your height, weight and blood pressure
  • Perform a physical exam
  • Discuss the need for health screening tests or labs based on age and personal and family history, including things like pap smears, blood tests, mammograms and more
  • Discuss immunizations recommended for you
  • Give advice to help you prevent disease, improve your health, and stay well

Depending on your health and personal risk factors, your preventive care schedule may differ from the standard recommendations. Talk with your doctor about a schedule that's best for you. If you have particular risk factors like a chronic disease, obesity, or a family history of a disease, additional screenings may be recommended.

A wellness visit does not deal with new or existing health problems. Please let our scheduling staff know if you need the doctor’s help with a health problem, a medication refill or something else. We may need to schedule a separate appointment. A separate charge applies to these services, whether provided on the same date or a different date than the wellness visit.

A Medicare Wellness visit is a specific type of wellness visit.

The term “physical” is often used to describe wellness care. But Medicare does not pay for a traditional, head-to-toe physical. Medicare does pay for a wellness visit once a year to identify health risks and help you to reduce them. At your wellness visit, our health care team will take a complete health history and provide several other services:

  • Screenings to detect depression, risk for falling and other problems
  • A limited physical exam to check your blood pressure, weight, vision and other things depending on your age, gender and level of activity
  • Recommendations for other wellness services and healthy lifestyle changes.

Before your appointment, you will fill out a form to ask about your health.

Why are immunizations important?o

It is extremely important to us that you have regular immunizations as scheduled to help prevent getting sick or having unnecessary health issues. We work to remind you of when it is time to have immunizations done. Remember to bring your immunization records. Up to date immunization schedules for adults and children are available at the Texas Immunization website.

What types of insurances are accepted at the clinic?o

We accept a wide variety of Insurance carriers. Go to our Insurance page to get a complete list of insurances accepted.

What is a Resident?o

A resident is an individual who has graduated from an accredited medical school and is obtaining three years of post-graduate training in the specialty of Family Medicine. Resident physicians work closely with experienced faculty physicians so that patients receive the benefit of up-to-date medical knowledge and experience in treating their conditions.

Why do I see a "Verify My Identity" link next to my name when I login?o

Our online account is not linked to your chart, hence you cannot make appointments online, see your chart online or request refills online. Please send us an online request using the General Comments Form or call us at (281) 325-4100 option 2 and we will immediately link your account to your chart. After your online account is linked to your chart, you will be able to utilize all functions of our website.

Why am I not able to make appointments or view my chart when I once was able to do so?o

To better serve our patients and ensure all patients who go online can fully utilize all areas of the website we are requiring starting Dec. 21st, 2011 for all users to verify their identities. We apologize for any inconvenience this may cause. Please contact us at (281) 325-4100 option 2 and we will immediately link your chart to your online account. To remedy the situation, we are also proactively linking charts to online accounts within 24-48 hours of patients registering or logging into our website.

How do I change my email address online?o

Your email address is tied to your chart as well as your online account. To change your email address, you must submit a request by completing our online Email Change Request Form. Once your request has been submitted, your email address will be updated within 2 business days. It is important to alert us immediately when your email address has changed. Also do not register with your new email address on our website. This will create another account and you will not be able to see your email messages from your doctor.

What is the Physicians at Sugar Creek Patient Portal?o

The patient portal is a tool created to give you 24 hour, anytime, anywhere secure  access to our office. The patient portal is only accessible by current patients who have been to our office. New patients must register to obtain access.
